Please contact Oxford Gait Laboratory with relevant details about the patient.
The following information is required for a referral to the Oxford Gait Laboratory:
- Patient name and address
- Patient date of birth
- NHS and MRN Numbers
- Diagnosis
- Name of referrer
- Date of referral
- Reason for referral (and question to be answered if known)
